Gender

• Gender denotes whether you are male or female
• Gender must be the same if shown on your form of ID
• If you wish to have your gender recorded as unspecified, please contact the Office of the USI Registrar by selecting Contact us
• Sex is another term for gender.

Email Address

• Email Address identifies where you want your emails sent
• The email address you provide must not have been used for another USI account

Mobile Number

• Mobile Number is the mobile number on which you may be contacted or receive SMS messages
• Cell number is another term used for mobile number

Home Phone Number

• Home Phone Number is a phone number on which you may be contacted
• If you do not have a home phone number, you may leave this field blank.

Authorise Mail Out

• Authorise Central West Courses to contact you via Email and/or sms

Preferred Contact Method

• You are required to provide at least one method of contact
• Preferred Contact Method may be email, mobile or mail

• State relates to the State or Territory where your address is located.
